New Client Intake and Health History Forms

The health and safety of our patients is a priority at the ACC. Once you have booked your initial consultation, you will receive a secure link to your New Client Intake Form and Health History Form via email. These forms can be completed at your convenience using your phone, tablet, or computer before your appointment.

Completing your forms in advance allows your chiropractic intern to review your health information prior to your visit and helps ensure your appointment runs as smoothly as possible. If you would prefer, you may print the forms, complete them by hand, and bring them with you to your appointment. Alternatively, paper copies can be completed on the day of your visit—please arrive a little earlier to allow sufficient time.

Please note that your initial consultation is an examination and assessment appointment only. During this visit, your chiropractic intern will conduct a comprehensive assessment, discuss their findings with you, and determine the most appropriate management plan. If clinically appropriate, chiropractic care will commence at your subsequent appointment.
